All registration for Northwest Catholic School takes place at the Lower Campus (7140 N. 41 Street).

Everyone needs to bring:

• Child's birth certificate
• Child's immunization record
• Northwest Catholic Registration Form

Milwaukee Parental Choice Applicants:

• 2011 Federal Income Tax Form 1040, 1040A, 1040EZ - signed and dated
• Evidence of City of Milwaukee Residency such as - current wage statement, current utility bill (water, gas, electric, cable or landline phone), current lease or property tax bill, or recent government correspondence (SSI, W2, TANF, Foodshare or Housing Assistance)

All Milwaukee Parental Choice Forms are provided at the Lower Campus Office.

Tuition Applicants:

• $200 tuition deposit
